Work in adult social care

Our adult social care teams offer a range of services to support local people in living fulfilling lives. Our teams include social care practitioners, social workers, and occupational therapists.

 

We work closely with health services, including primary care, mental health, and learning disability teams, as well as our own housing and communities teams and the community and voluntary sector.

We are adopting the three conversations model to ensure our practice is strengths-based, relational, and personalised.

 

Our service is made from:

 

  • long-term conditions teams
  • integrated learning disability teams
  • mental health teams
  • a preparing for adulthood team which supports young people aged 18 to 25
  • specialised teams for adults with autism
  • approved mental health professionals

  • a personalised induction with online and in-person training, shadowing, and mentoring
  • a safe, manageable workload with continuous support
  • continued professional development (CPD) with training and reflective practice, in partnership with Bournemouth University
  • regular one to one supervision and reviews for professional growth
